really useful breakdown of the differences between the two, especially why tb-500 handles systemic tissue remodeling while bpc handles more localized repair. that distinction gets glossed over in a lot of content. ran the wolverine stack through formblends after a partial achilles tear and the sequencing question (which to start first, how long to overlap) was the thing i couldnt find a straight answer on. if you have a follow-up video on timing protocols specifically that would fill a real gap.

really good breakdown on the dosing ranges. one thing i'd add from my own experience is that body weight isn't the only variable worth factoring in, injury severity matters a lot too. i was running 250mcg twice daily through formblends for a partial achilles tear and my provider actually bumped me to 500mcg once daily instead, said the single larger dose worked better for tendon tissue specifically. took about 3 weeks to notice real changes in morning stiffness.

I (52f) had very similar issues. My MRI showed a partial supraspinatus tear, a bone spur and all around degeneration. After 1 cortisone shot that only helped for a few weeks and some PT, my Dr. recommended surgery as I was in a lot of pain and had very limited ROM which affected my ability to do my job (catering/events). Well when they went in they found a labral tear and my biceps tendon red, angry, & hanging on by a thread. So I ended up with biceps tenodesis (2 screws), labral repair, decompression & bone spur shaving. He actually left the partial tear because he said that wasn’t the problem. I don’t regret having the surgery because my biceps tendon wouldn’t have fixed itself, but I will say it’s been a harder recovery than I expected. Surgery was 3/20, started PT at 2 weeks post op. I am still not clear for work. I had open Achilles repair last year and the recovery & rehab was easier. I am pretty fit but I think my body is telling me to go back to a desk job!

The promoting effect of pentadecapeptide BPC 157 on tendon healing involves tendon outgrowth, cell survival, and cell migration.

BPC-157 was found to accelerate the outgrowth of tendon explants, increase cell survival under stress, and promote the migration of tendon fibroblasts. This is likely mediated by the activation of the FAK-paxillin pathway.

This study found that BPC-157 improved functional recovery of the Achilles tendon to bone unit after transection in rats by reducing inflammation and promoting new blood vessel formation. BPC-157 outperformed methylprednisolone in this regard, as it did not suppress new blood vessel formation.

Achilles detachment in rat and stable gastric pentadecapeptide BPC 157: Promoted tendon-to-bone healing and opposed corticosteroid aggravation.

The peptide BPC-157 was shown to improve tendon-to-bone healing in rats with Achilles tendon detachment, and also reduced the aggravating effects of corticosteroids on healing. This suggests that BPC-157 may be a useful treatment for promoting tendon and bone healing.

The study found that BPC-157 accelerated healing of transected rat Achilles tendon and stimulated tendocyte growth in vitro. The peptide improved recovery by increasing load of failure, functional index, and promoting collagen formation.
